Helo all,
We are currently in a project involving data aquisition to a PC with the PIC 18F4550.
Data is send to the PC via USB port but PC must see a virtual comm port. Microchip offers standard library, CDC for doing so. Standard Windows driver is also used.

As far as I know with a careful design, phase noise in a VCO is dominated by current source. I have seen a couple of designs were very large transistors are used in the current source to avoid flicker noise influence in phase noise.

The list of non idealities is large, your question seems too general but do not forget to consider if some of the following affect your circuit:
- Charge injection
- kT/C noise
- charge time to your capacitor if your sampling rate is too large
- offset at your buffer...

Stand-by probably sets the output in tri-state, more correctly referred as high impedance. At this state the node's voltage is not driven by your IC (an output with an extremely high impedance).
Also CMOS compatible nowadays has little meaning. It is just that the output...
Grounding strongly depend on your application so specify for an accurate response. Usualy inside an IC you provide both VCC and GND lines as thick as possible, however for example in a micropower application this loose sense.
As far as I know is not a usual practice to provide a full gnd plane...
