Wiring standards
Are there any documents detailing wiring colour coding ie. what colours are used for signals, power , transducers etc.

The reason that I suggest seperate supplies is to isolate the logic from the power circuit. As soon as a heavy load is switched even with RC snubbers,
varistors etc. the circuitry will start to fail again! Try switching 10 amps/120VAC RMS...
The optocoupler only improves the coil to contact...
Use a seperate supply to drive the relay coil and use an optocoupler to drive the BJT. Do not forget the free wheeling diode to supress inductive kickback ( across the BJT collector to the relay positive supply as previously mentioned). The return ground(s) for the logic circuit and relay

fr4 leakage
Possibly because the Er dieletric constant for FR4 varies widely. Teflon is used for high speed signals design, closely matched impedance control traces can be designed.
