what does that mean?
 

lambda refining mesh

probably due to the drawing of your model, i hv encountered the same problem before. Try to redraw your model or change the mesh size

yor frequensy may be extra-high, or dimension too large..
 

you mean HFSS ha?
It may also arise because of a non-smooth transition. make your contacts as smooth as possible. for example when a sphere and a large cube intersect
