What factors lead common choke burnt in a flyback converter?

Hello,
Recently I found the common choke (L801) was badly burnt in my flyback converter circuit. At same time, two capacitors (C806&C813) were also blown. In fact, I found this problem in only two out of 50 such kind of boards which were running under the same condition. I can guarantee these two bad boards were not overloaded and had been working soundly for a while (about 10 days).
I am wondering what factor would lead such failure. Can anyone help me on explaining this problem? The schematic is attached. The input voltage VSYS+ is 24VDC from preceding AC/DC power supply.
