Terminal connection of Antenna diode

Can you please help me out that for fixing antenna violation which terminal of the dioade is connected to ground and which to effected pin?
 

In substrate: anode to GND, cathode to gate.
In n-well: anode to gate, cathode to VDD.

Like the ESD protection diodes at chip I/Os.
