Question in symbolism

This is a symbolism I found in a book...I'm talking about the first mosfet.Just ignore the other one..

My question is where is the bulk(body) supposed connected to in this symbolism for the p and n mos?
 

Another similar question....I was looking at a book with the same symbolism as above and i assumed that the bulk was always connected to the source.Yet at some point i saw this....

Why the difference here?if the default in the symbolism is for the bulk to be connected to the source why is there a difference here?
