Problem with WIMAX Simulink module ,,, please help me

I am simulating the physical layer of WIMAX on Simulink using a 16 QAM only.
There is problem with my module that appears as different states between the input and the output. I need help with the error in the simulation.
the simulink model is attached. thanks for trying to help.
