Why there are two C connected together, you should ask the manufacturer.
How to connect t depends on your application. Since it is a mechanical switch there is no dedicated Input or output.
You may connect 5 different audio sources at the pins 1..5. Then feed C to an amplifier...
Or you may connect C to GND. And connect your load between VCC and the pins 1...5.
If your switch is in position 1, c and 1 will be connected together, in position 2, c and pin 2 will be connected together and so on. If you don't fully understand this, you may want to post your circuit so we can tell you exactly what pin on your switch connects to which point in your circuit.
