Looking for ProMate ICSP 15 pin connector pinout

I seemed to have lost my ICSP cable that attaches to the ICSP adapter of my ProMate II. I can make a new cable if I knew the pin-out of the 15 pin connector of the ICSP (it looks to be a 15 pin VGA connector). Microchip and every other source I have searched does not seem to have this information.

Can anyone provide a pin-out?
