I am replying to a kind of old post, but just in case you are still looking for an answer... It is amazing how even the simplest circuits can go wrong (I have plenty of experience with this phenomenon!). The key is to check everything and make no assumptions. Is power and ground on the right pins? Your schematic doesn't show power. Can you see a signal (assuming you have a scope) on the input? Are the inverting and non-inverting inputs at the same potential? Are you sure you have the right value resistors? If you still can't get it to work, pm me and I'll look at it.
