How to use diode to solve antenna violation?

I have two antenna violation in my layout. Long metal is connected to pmos gate. I plan to use reverse biased diode to solve the problem. how should I connect the other terminal of the diode?
If I use N+/pwell diode, should the other end connect to ground ?
 

Yes. Diodes should not be floating.
 

Yes , thats the correct way to connect an antenna diode...but if possible avoid usage of diodes will generate unnessesary parasitics in the circuit use jumpers wherever possible.
 

always connect the diode in the inverse biasing. be careful about it or your circuit will fail for conduction. so n at Vdd an p at ground.
