how to filter out a voltage spike?

If there is a voltage spike in the useful signal, the duration of this spike can be between several hundred ns to a few us. The period of the useful signal is 1ms

usually how do people remove this voltage spike in the circuit? I want to build a simple filter by my own, this filter will only filter out this voltage spike but leave the useful signal
 

use the median filter , it is easy implemented in a digital form.
