How to create the lef def layer map and gds layer map files?

How to create the lefdef layer map and gds layer map files?

How Do i create these twwo types of layer map files? My streamOut for some reason only includes the gds numberings for metal1 onwards. Doesn't include nwell, pwell, poly and so on. Can someone please advice?
