How can i program 12f series with ICD2?

Hi,
How can i program 12f series pic mcus with an ICD2 and PICDEM2 plus demo board?
 

is there a shematic of that adapter? Can i build it?
 

It may be more simple to solder a modular cable directly to a 8 pin socket. That's how I programmed 12F chips.

Signal..Modular..Cable..12Fxxx
VPP.....1............bl........4
VDD....2............ye.......1
GND....3............gn.......8
PGD....4............rd.......7
PGC....5............bk.......6
N.C.....6............wh
