Ethernet MAC and PHY, different manufacturers?

Hi guys,

Are there different manufacturers for the two parts of a Ethernet network controller?
It's strange for me to see ethernet MAC inside SOC and ethernet PHY on Motherboard and not as a whole IC. Why there are two separate parts?

Also, this controller(MAc+PHY) can it be in any of these thress forms?

1-Integrated on the same DIE as processor(SOC)?
2-integrated on the Motherboard?
3-MAC part integrated on SOC but PHY integrated on Motherboard?

Why do we have these different combinations?
