Design DC/DC and AC/DC power supply

Hello,

How would I design a power supply that takes in 12V AC/DC input and output 12V DC for an embedded application?
 

Simplest is a bridge rectifier and a filter capacitor. It changes the 12vACrms to ~15.5vDC, or 12vDC to ~10.5vDC.

But:

Does the AC/DC supply have a "common"?
Does it need to be a regulated 12V out?

More details on the possible power sources and what the "converter" is powering please.
