9945 is almost certainly a date code.
The logo looks like siliconix (Vishay)
T512 is probably shortened from a longer part number.

What application is it used in?

Hello - almost forgot about this. I'm not sure what it is used for. Someone where I used to work gave me a few dozen over a year ago. Long since forgot who, or where I wrote the full part number. I only remember thinking I should keep them.
