I want to design a circuit that has an input of 4-20 mA(loop power) and 4-20 mA output with input and output isolation

I believe that this is your task, I'm not going to provide you a ready circuit, I have just pointed you to a component you can use to achieve what you want.

Check the datasheet example and modify it to your needs.
