looking for filter. coax to RJ11 in cable TV

I have modem router with RJ11 input. The cable TV provider gave me modem with coax to RJ45
that gives direct connection to PC. I need a unit without the modem only the filter with coax to
RJ11 (Phone connector) that will allow me to connect my modem router to cable TV.
can you help me to find it?
