A small disk with a fin or a small propeller sit in the bottom of the glass. In the base there is a small motor spinning a magnet around. The rotating magnetic field drags the disk around and stirs the water.

Is it really something we need to place inside the flask ??

the didn't anything like that . . . please check this link . . .
 

I looked at the link, it very clearly shows a white 'strirrer' in the bottom of the flask and it even 'explodes' the base contents where you can see the motor.

So yes, you do need to place the disk in the flask. It has no physical connection to the motor, it has a magnet embedded into it that the base motor drags around to get the motion.

... and all the medical and chemical claims it makes are completely false!
