L6699 IC not starting =(

I made a circuit(see attached sch).
Half bridge MOSFETs not drawn.

IC L6699D do not starting.
Pin 11(LVGate) = 15V
Pin 12 (VCC) = 15V
pin 1,4,5 = 2V
pin 7 = 6V
pin16 (VBoot) = 12V
Others pins = 0V

Oscillator (CF pin 3 = 0V) do not starting.

What should I do?
