Help me design an analog level shifter

I need to design a voltage level shifter from dual supply input to single supply output. When input is -12V ,0V & +12V then output should be 0V,+6V & +12 respectively. Any recommendations on the circuit design for this level shifter?

A voltage divider (two resistors) between +12 and the input, perhaps an opamp buffer on the output.
