PIC18F452 three phase inducion motor controller out put fluctuation

Hi All
I have completed AN843 based 3phase motor controller and its working forward direction
is perfect, but when the reverse function, output fluctuating and fault light also blinking.
when I touch the reverse button or negative side of PIC18f452 micro controller the out put goes off. please suggest any cause ..
