Frequent damaging problem with IR2110 gate deiver for Full Bridge

I was using IR2110 as MOSFET drivers for my quasi-sine wave inverter. I was using continuous logic signal. pin 9 is given 5V as i'm using 5V logic, VCC=12VDC, CB= 2.2uF, D = 1N5817 . But the problem is the driver is damaging frequently and also creating short ckt to the MOSFET path. Also MOSFETs are damaging frequently even without any load. But if I remove IR2110, no damage occur.


Can you say why this happen??
 

I'd begin by looking at the bootstrap node and the high side
source node. The diode reverse recovery time could pull back
too much charge from your fly-cap, fly-cap could be too small
for the operating frequency, etc. You say continuous logic
signal, bootstrapped half bridges need finite off time to charge
the fly-cap and have a maximum on time before it bleeds dry.
