How to isolate a HART modem circuit from the 4-20 mA loops?

Can anybody tell me how to isolate a HART modem circuit from the 4-20 mA loops.
 

HART

I use transformer EMIT1200 or SM-LP5001 plus capacitor 0.47 uF - connect parallel of HART-line wires. For uncharge meet the resistor (100 kOm) after capacitor.
