you can model the varactor as resistance + capacitance. you can use two RLC components in series in HFSS. that's how we model varactors.

you cannot place a waveport inside your geometry, unless it is backed with PEC. therefore, waveports are usually placed at the outer boundary of the model (which is backed with PEC by HFSS automatically).

you can simply use the cavity model for annular rings... look in microstrip antenna design handbook, or any other microstrip antenna book...
this usually happens when the dielectric materials are not assigned any losses. also most probably radiation boundaries are not meshed as good as the dielectrics and efficiency is calculated wrong...
