I need minim Power supply +-5V for example AD8638 have noise 66nV/Sqrt(Hz).

You consult amplifier, that would be comparable to or better than the LT1124. In particular, the fit slightly lower input current ~1 - 2nA.

- +-13V power supply, I have +- 16V for other amplifier
- Input Bias Current datasheet say 10pA on 25C, I measure 16pA on 22C, hundreds of fA up to 1 pA would be better
