My dear how can you go with choose a cutoff frequency of 1000Hz when you have a 400Hz input waveform. If you do so what is the necessary of using the filter
Bias winding is used for powering the control circuitry where as Secondary winding is required output of the converter, so for regulating the output voltage we use an opto-coupler to isolate the secondary from primary winding.

Have you verified the gate pulses of all the mosfets .i.e have you verified the boostrap circuit? The leakage inductance of M3 mosfets seems to be higher that's the reason it fails more often. plan with a series capacitor as you mentioned. and also the layout need to be modified for...
