The processor is atmega328p microcontroller.
There is a problem with the "stop" method of "pwm" class. In the stop method only clock source has been disconnected from PWM but the ports are not reconnect to GPIO. I have changed the code and now it's working fine.

Why you are using the old buck converter design by using expensive and bulky inductor. Unless you don't want much power from the buck converter and you have the arduino why not use the inductor less design with feedback for your constant 12Volts output.

It seems you are using PWM for dimming the heating element.
If the element is on AC and you put an LED parallel with the Heating element, you must have to very careful. Because LED will glow fairly at 20mA with forward voltage of 1.28Volts (most LED), at 240V rms you will have to use 16936 Ohm...
Try increasing the output impedance which will lower the current and OP-AMP will not heat up.
If the OP-AMP in any circuit is heating, the circuit or PCB or both have problem.
