simplest way to use a balun in simulation is to use a quarter wave sleeve on the coax (a copper strip lembda(guided)/4 in length and shorted to outer of coax) to block the ouer shield current due to balanced to unbalanced transition.

Can anybody guide me how to model diode in HFSS? I am not sure if it can be directly done using HFSS but is there other way that i can do it? I want to model diode to use it as switch in my antenna design.

There are many tools available such as HFSS, CST, WIPLD. HFSS and CST are easy for modelling structures. HFSS is widely used and is FEM based which you can try.

You can get circular polarization without going for truncated option by exciting two orthogonal modes of a rectangular patch antenna. Use a 90 degree phase shift for other using a branch line coupler and then excite the two modes (one along width and other along length). Hope this helps...

A very big air box size or may be if the structure is large then a very fine mesh sometimes create problems. You may try to adjust these parameters and then go for analyzing the problem.
