Could you upload pdf version of pcb (or sources from Eagle)
 

icd2 zif

ZIF adapter for programming is quite troublesome.
Why not try programming using ICSP interface.?
Easier and save alot of time.

I don't give away the PCB files but the schematic is available on my site for a ICD2 ZIF adapter "Firefly."
