Driving Point function and Transfer function

HI there,
we all know that all the zeros and poles of a driving point function is the pole of transfer function of a network, i just wonder is there any formula for how to manipulate those poles in a transfer function to get a driving point function? E.g. Soppose i know the transfer function T12 from port 1 to port 2, now i want to find the driving point function D3 of port 3, i know all the zeros and poles of this driving point function come from the poles of the transfer function T12, but which pole should be moved up to be zero, which not? I guess there are some formulas that can give the answer.
