Power Transformer for Light Switches

Hi guys,

I was checking the power supply of a light switch and I found it as below.
A virtual ground was implemented by the circuit below, but I don't understand how it was done. Especially the transformer; it has 4 terminals on one side and 3 terminals on the other. 3 terminals can mean center tab transformer, because the transformer does output 2 levels of voltage- namely 3.3v and 12v- besides the ground. On the other hand, the other 4 terminals of the transformer are ambiguous.
What are these terminals?
What is the model- or family of models- of such a transformer?
Where can I buy such transformer?

thats a switching mode power supply circuit and I guess its a fly-back type (I see pc817 optocoupler).
the transformer is ferrite core switching transformer with two output winding
the transformer is specially designed for that circuit
you can find design information in switcher IC datasheet or application notes
