PWM signal destroyed the inverter?

Hello everybody!

I just made up an circuit board controller based on MC3PHAC, designed for AC motors. Everything is controlled by PWM signals(6 of them), from the MC3PHAC to the inverter. Everything went just fine unless something strange happened when I made a 3 phase redressor circuit for the inverter. Somehow when I started the controller(the load was low on inverter, as I used a low power ac motor). the inverter just blew up, resulting that two of the three arms of the inverter IGBT's were destroyed. Now I am asking that if the bad connections of PWM could result into such thing, because I realised later that one PWM outpun of the MC3PHAC was in a bad contact with the board, and also the signal wasn't well trasmited to the inverter. Thanks guys! Waiting for some advice.
