Buck Converter Burned

Hi Friends,
In my board, i have may buck converters such as +12V to 5V, 3.3V,1.8V,1,V.
But my +12V to +3.3V Buck converter burned in between of its operation (not on first power-ON), i checked that the UVLO & OVLO of Buck converter, its 5V & ~ 14V, absolute max rating is 16V.

Do you have any idea on the root cause of the burning?
Another question, why this buck converter (+12V to +3.3V) burned, why not others? In order to verify this, what are factors do i need to check apart from OVLO?

give more details. part numbers, schematic. something to answer a technical qtn
