Thanks for a very good forum!

Is it possible to make a STB ( IPTV ) with multi room solution ? With multi room solution I mean as this example.

John and Anna are living together. They have one IPTV STB at home and are often fighting for the remote control because they want to watch different channels. My question is, is it possible to make an IPTV STB that can allow watching two different channels at the same time in two different TV ?

For example the STB is connected to the TV in the living room and John is watching a football game. Since the STB allow to watch two channels, Anna has choosed to watch BBC on the TV in the kitchen.

What's the difficult part in this scenario ? Is it to make the STB play two channels ? Or it's transporting the video of the second channel from the STB to the TV in the kitchen ?

If the problem is transporting the video, could it be solved if they connect an AV link to the first STB ?

Certainly Multichannel streaming from your "headend" ISP is possible, but being a fee-based technology & service only your ISP knows for sure.

In the meantime, there is XBMC software and XBMC HUB with addons for any computer, for recorded video, OTA (Over-the-air)with a digital tuner to router e.g. HomerunHD ASTC digital tuners from https://www.silicondust.com/ and then there is negotiated timeshare.  There is always a solution somehow.
