failure in source mapping - hf3d error

Has anyone encountered this error before? I can provide more information as needed. Apologies if unclear, I'm new to this.

[warning] PML Setup: PML Material parameters are out of sync with the problem setup and need to be recalculated using the PML Setup Wizard.
[error] Adaptive solution setup, process hf3d error: Failure in source mapping.
[error] Simulation completed with execution error on server: Local Machine.
