[SOLVED] Ambiguous Encoder output

My servos are giving encoder output in this series --> 10 11 01 11 10 11 01 11 ...
What could be the possible fault ?
 

are you sure its not : 01 00 10 00 01 00 ... ?

I too thought it would be so, but the encoder position was not reflected as expected. So when verified the encoder output it was 10 11 01 11 ....
Could you suggest me any possibility for such kind of error ? ...
 

The strange part is not the sequencing, but rather the fact that there are only 3 unique codes.
That seems to suggest that something is shorting or is open-circuit.

Could it be because of low pull up resistance ??

This is my servo.. Now with such encoders response, How do I find the sense the direction of rotation ??
