QPSK noise addition help simulink

Hi all,

I made a QPSK mod/demod in simulink which AWGN channel. The output is fine when it comes to seeing random bits. I also dont want to use inbuilt QPSK/AWGN blocks. But the noisy scatter plot doesnt come out to be ok. Can anyone help me on this? Also, please guide me on making a BER/SNR curve from this model(attached).
