Problem in designing Headphone amplifier using NE5532

HI Guys,

I am designing a headphone amplifier using NE5532 IC.The schematic is attached below.

But here i encounter a problem that the amplification is not good and there is a terrible type noise at the output with the audio.
Guys plz suggest me some solutions regarding this .
Also i wanna make it in stereo and wanted a PAN controll for this.
 

what is the input signal level ?

bye the way, why you need such high gain for your headphones. please ensure that such gain wont burn off the headphone coils.

try to shift the c2l to between R2L and opamp pin.
