how to use wrsamp iin converting txt file into WFDB signal file format 16

According to the guide and example,I try to convert my txt file into WFDB signal file format 16 .But I failed
I tried many times by changing parameters,still not succeed.
Thanks for your help! The txt file is attached as an appendix .
