Microstrip Antenna With IE3D

Hi,
I am designing a 915Mhz microstrip antenna, and I am trying to simulate results using IE3D.
I tried to simulate the two attached designs from Texas Instrument.
However I am not sure how to define the ground planes, should I set layer 0 conductivity to 0 and draw a ground shape? what about the ground polygon on the top layer, how can it connect to layer 0.
Another thing is the feed port, should I create a cutout in the polygon because the tool complains if I don't
If someone can give me some explanation, or if he can build one model for me I will be great full.
