3 phase failure detector

Dear All

i am designing a system for phase failure alarm in a 3 phase system
the system will detect the failure of any or all the phases and start an alarm
can anybody help me providing the schematic for the detection of phase failure circuit? i am using a MCU for the alarm side and it's working fine when i test with logic inputs for the phases. how i can detect the phases of the 3 phase line?

your word failure may be too big to understand ,because failure means absence of one or two/three phase completely/ or one or two/three phase under voltage below 70% or over voltage 125% rated voltage so which system do you want pls reply.

the word failure means is the absence of one, two or three phase supply.
i think the schem that you send will work for me.

To detect all cases of failure, a professional voltage monitoring relay measures three interphase voltages, optionally the star point voltage against neutral and possibly phase sequence. Depending on the equipment to be protected by the voltage monitor, the said effort may be required to prevent damage caused by voltage failure.
