You have actually got two errors there -- one about not finding the matching subcircuit in the file, as ErikL noted, and one about cannot read source. I suspect the second error can help you figure out why the subcircuit match was not found.
To find out which source is not being read, take a look at the LVS REPORT file. Assuming the 2011 interface is the same as the 2017 interface, the file name is in the Calibre Interactive - nmLVS window, in the Reports/SVDB tab (Outputs pane). It should be in the directory you started Virtuoso in. The report is pretty detailed, and described in the Calibre Verification User's Manual (calbr_ver_user.pdf) in the "LVS Results" chapter.
