placing of patch antenna on ebg mushroom

Hi i am bit confused about the ebg mushroom type antennas. how the patch is placed on the ebg structure. is it (patch+substrate without ground) is placed on the ebg structure which acts like a ground. or we make the ebg structure and remove some of the cells so that patch can be placed. in this case patch and ebg substrate are common and all the metal layer(patch + ebg cells) are in same plane.

so they also keep the whole patch(patch+substrate+patch ground) directly on top of ebg. in this case the ground of patch will be in touch with the ebg surface metal. take a look at the paper i've attached.
or did they removed the metal from the place where patch was fabricated, and if they did how to calculate the metal clearace area so that patch can be placed.
