driver Ir2112 burning

hi,
I'm trying to control a H-bridge with one IR2112 driver, the circuit works, but burning driver with few uses. I am using P channel Mosfets on and channel Mosfets N down and not have the bootstrap circuit will be necessary?
i will appreciate any suggestion or critics, thanks in advance ponte h.png
 

I would rather fear shootthrough and output transistor burning... In this case, the driver might be damaged as a consequential error.

What's Vdc level?

Vcc must have bypass capacitors, D1 should be replaced by a short. Exceeding Vcc rating would be my first guess if the output transistors are still healthy.
