Is this DC to be at a high current? If it is only a signal level, you can use the switched capacitor filters from Analog Devices which can be set to very low cutoff frequencies. You may need to preceded one by a LC type filter if the AC component of the source is high in frequency or voltage.

If the DC is high current you are stuck with the LC type filter.
