problem with DTMF decoder...

hi im working for a project of epabx....... n i've problem with dtmf decoder....all circuit components r assembled correctly..but pin no.11,12,13 n 14 r always high while pin no. 15 is always low...im unable to find what the problem is....i m in search of solution...
