i am using orcad for schematic entry
pads for library design ,importing netlist ,some placement ,
specctra auto router for placement and routing
bring back design to pads by using specctra translater
generate final gerber in pads
for signal intigrity use cadance hyperlynx ,specctra quest...

if rise time time is fast then rise time length of signal routed reduced
if routed lenght is more than rise time length then route acts as transmission line

generally gap between plane split taken is 20 -30 mil
if gap is more then noise coupling between plane split is reduced ,
but more gap create problem to signals crossing over split .

pcb quality depends up on no of factors
1) whether manufacterer able to manufactor board with proscribed design rules
like clearance between wire to wire ,wire to pad ,wire to via ,via to via etc ,
2)min trace width with which he can manufacture
3)min diameter via he can...
bypass decoupling capacitor difference
but generally i have seen bypass capacitors are used in series
and decoupling capacitors aare used in parallel
