Is it simply by integrating the volume loss on a dielectric or metal layer?
How is expressed the output value ? in dB ?

What says jiboune256 is true if you allow the solver for more than 1 mode. It is on the settings of the waveport.
So if you need to see of higher modes propagate, do not forget to increase the number of solved ports.
